This paper was presented as part of Business Session 1, on the topic of 'Ongoing issues of safety quality and security in world shipping', which was chaired by Peter McQueen.

This paper contains the slides for McCarthy's presentation responding to issues arising from Captain Botterill's paper. In particular, McCarthy focuses on a case study of the MT 'Erika', and consequential changes to the safety network in the shipping industry.
